About the Position

As a Home Health Licensed Practical Nurse, you will provide skilled nursing services to patients in their homes under the direction and supervision of a Registered Nurse or other authorized clinician, as required by applicable regulations and the patient’s plan of care.

You will help patients manage ongoing health needs, reinforce education provided by the care team, monitor changes in condition, and communicate important clinical information to support safe, coordinated care.

This role is ideal for an LPN who values compassionate nursing, enjoys working independently in the field within their scope of practice, and understands the importance of treating every patient with dignity and respect.

Responsibilities

• Provide skilled nursing care to home health patients in accordance with the established plan of care, physician orders, and applicable regulations.
• Monitor and document patients’ health status, symptoms, vital signs, and response to treatment.
• Administer medications and perform nursing treatments within the LPN’s authorized scope of practice and applicable orders.
• Assist with wound care, disease management, and other nursing interventions as assigned and permitted by licensure.
• Reinforce patient and caregiver education regarding medications, treatment plans, symptom management, and home safety.
• Recognize changes in patient condition and promptly communicate concerns to the supervising Registered Nurse, provider, or appropriate member of the care team.
• Complete accurate, timely clinical documentation in accordance with agency policies and home health requirements.
• Collaborate with Registered Nurses, therapists, physicians, and other members of the interdisciplinary team to support continuity of care.
• Promote patient dignity, safety, comfort, and independence throughout every visit.
• Follow infection prevention practices, patient privacy requirements, and applicable clinical standards.

Required Qualifications

• Current, active Licensed Practical Nurse (LPN) license valid in California.
• Valid driver’s license.
• Reliable personal vehicle.
• Current automobile insurance.
• Ability to travel to patients’ homes throughout the assigned service area.
• Strong communication, organization, and patient-care skills.
• A compassionate, dependable, patient-first approach to nursing.

Home health experience is a plus. Candidates with a strong commitment to quality care and a desire to serve patients in the community are encouraged to apply.
