Job summary

Caringhand Home Health Solutions, Inc is seeking a field-based LPN/LVN to provide direct patient care in homes across California. You will implement care plans, educate patients, and collaborate with RNs and other clinicians to support safe recovery.

This per-diem role requires CA licensure, CPR, a valid driver's license, and 3+ years of nursing experience; a strong, patient-first mindset and reliable transportation are essential.

Qualifications

• Graduate of an accredited practical or vocational nursing program.
• Active LPN or LVN license in California.
• 3+ years of nursing experience; home health or med/surg background preferred.
• Current CPR certification with AHA.
• Valid driver’s license and reliable insured vehicle; ability to travel to patient homes.
• Strong communication skills and a patient-first mindset.

Responsibilities

• Provide direct patient care in accordance with the state Nurse Practice Act.
• Implement and follow individualized care plans established by the RN.
• Educate patients and their families on self-care techniques and diagnosis-related information.
• Document care accurately and in a timely manner.
• Collaborate with the care team — including OT, PT, HHA, and other disciplines — and communicate patient needs to the RN or Clinical Supervisor.
• Use supplies and equipment safely, adhering to aseptic technique.
• Perform other duties as required for patient services.

This is a field-based, per diem position that requires travel to patients’ homes. Although listed as “remote,” this is not a work-from-home or telehealth role. Compensation is $45–$55 per visit.
