Job summary

Amador Home Health in Jackson, CA is seeking a Home Health Physical Therapist (PT) for full-time in-home care. You will provide skilled physical therapy services to patients in their homes, improving mobility, strength, balance, and independence while delivering patient-centered, evidence-based care.

You will work independently in the field and collaborate with a broader home health team to coordinate services and support positive outcomes. A valid PT license and driving requirements apply.

Responsibilities

• Document evaluations, treatments, progress, and other required clinical information accurately and in a timely manner.
• Collaborate with physicians and other members of the home health care team to coordinate patient care.
• Follow applicable clinical standards, professional guidelines, and home health documentation requirements.

About the position

As a Home Health Physical Therapist, you will provide skilled physical therapy services to patients in their homes. You will help patients improve mobility, strength, balance, functional independence, and overall quality of life while supporting safe recovery in a familiar environment.
