Job summary

Caringhand Home Health Solutions, Inc. is seeking a Licensed Practical/Vocational Nurse to deliver direct patient care in clients' homes, implement individualized care plans, and educate patients and families on self-care.

This is a field-based, per diem role requiring travel to patient homes (not remote). Compensation is $45–$55 per visit, with flexibility to convert to full-time based on performance; benefits include paid sick days, bereavement leave, and mileage reimbursement.

Qualifications

• LPN/LVN license in California required.
• 3+ years nursing experience; home health or med/surg preferred.
• Current CPR certification (AHA).
• Valid driver’s license with insured vehicle and ability to travel to patient homes.
• Strong communication and patient-first mindset.

Responsibilities

• Provide direct patient care per the state Nurse Practice Act.
• Follow RN-established care plans and educate patients and families on self-care.
• Document care accurately and timely; collaborate with care team.
• Use supplies safely and adhere to aseptic techniques.
• Perform other duties as required for patient services.

Responsibilities

• Provide direct patient care in accordance with the state Nurse Practice Act.
• Implement and follow individualized care plans established by the RN.
• Educate patients and their families on self-care techniques and diagnosis-related information.
• Document care accurately and in a timely manner.
• Collaborate with the care team — including OT, PT, HHA, and other disciplines — and communicate patient needs to the RN or Clinical Supervisor.
• Use supplies and equipment safely, adhering to aseptic technique.
• Perform other duties as required for patient services.
