Job summary

Amador Home Health is seeking a licensed Physical Therapist to provide in-home care in Jackson, CA. You will deliver skilled PT services, promote mobility, balance and functional independence, and work with an interdisciplinary team to support positive outcomes.

The role offers competitive per-visit pay plus SOC compensation, driving reimbursement, and comprehensive benefits. You will travel to patients’ homes, exercising clinical autonomy while prioritizing patient-centered care.

Responsibilities

• Document evaluations, treatments, progress, and other required clinical information accurately and in a timely manner.
• Collaborate with physicians and other members of the home health care team to coordinate patient care.
