Job summary

A-Line Staffing Solutions is hiring a Licensed Practical Nurse to provide direct nursing care in a rehabilitation environment. You will perform patient assessments, administer medications, and collaborate with RNs and other staff to support patient recovery.

Shifts are 6:45am–3:15pm or 2:45pm–11:15pm, five 8-hour shifts per week with every other weekend included. Benefits include after 90 days, plus a 401(k) with company match for full-time employees.

Qualifications

• LPN license required in Pennsylvania.
• Experience providing direct nursing care in a rehabilitation environment.

Responsibilities

• Provide direct nursing care to residents/patients in a rehabilitation environment.
• Perform patient assessments, monitor changes in condition, and report concerns to the RN and provider.
• Administer medications and treatments according to physician orders and facility protocols.
• Provide wound care, collect vital signs, perform clinical treatments, and assist with daily nursing needs.
• Document patient care, progress notes, medication administration, and assessments accurately in the EMR.
• Assist with implementing individualized care plans and support patient recovery goals.
• Communicate effectively with patients, families, physicians, and interdisciplinary healthcare teams.
• Collaborate with RNs, nursing assistants, and rehabilitation staff to ensure quality patient care.
• Assist with admissions, transfers, and discharge processes as needed.
• Maintain infection control standards, safety practices, and compliance with facility policies.
• Provide compassionate, patient-centered care while promoting comfort, dignity, and independence.
