Job summary

Genesis Home Care in South Zanesville, OH is seeking a skilled LPN for in-home visits Monday–Friday 8:00 AM–5:00 PM with on-call requirements. A $7,500 sign-on bonus is offered.

You will provide skilled nursing care, assess patients, administer meds, and educate families on disease management and safety in the home setting. The ideal candidate has an active Ohio LPN license, 1 year acute care experience (2 years preferred), and home health exposure.

Qualifications

• Current, active LPN license in Ohio.
• Minimum 1 year of acute care LPN experience; 2 years preferred.
• Home health experience preferred.
• Strong communication and interpersonal skills.
• Compassionate, patient-focused approach.
• Ability to work independently, prioritize responsibilities, and make appropriate nursing judgments.
• Strong attention to detail and reliable work ethic.
• Ability to work effectively as part of a team.

Responsibilities

• Provide skilled nursing care according to physician orders and RN-developed plans of care.
• Assess and monitor patient conditions and communicate changes to the RNCM.
• Administer medications and treatments as prescribed.
• Educate patients and families on disease management, medications, safety, and preventative care.
• Involve patients and families in their plan of care.
• Complete accurate and timely clinical documentation per agency and Medicare requirements.
• Communicate with the care team and scheduler regarding patient needs and schedule changes.
• Participate in on-call coverage, case conferences, in-service training, and required meetings.
• Provide care within LPN scope and state/federal regulations.
• Travel to patients' homes within the assigned geographic area.
