Job summary

CareRing Health in Ohio is hiring a Home Care Nurse (LPN) to oversee care for clients in their homes. You will administer medications, document care, and collaborate with the clinical team to implement the Plan of Care.

Starting pay is $27 per hour, with a Thursday 11 PM–7 AM shift and differential offered. Benefits include health insurance, weekly pay, and a flexible schedule.

Qualifications

• Valid State LPN License.
• Valid driver's license and use of automobile is required due to the travel involved.
• Annual MUI (Incident) training as dictated by Ohio Medicaid and related programs.
• Must read Plan of Care and various agency documents.
• Must be able to use a computer, laptop or tablet.

Responsibilities

• Administer medication in accordance with standards of practice.
• Encourage rehabilitative skills and self-care independence.
• Perform client treatments per Plan of Care.
• Assist client in learning self-care techniques.
• Prepare clinical notes documenting care provided.
• Monitor and report changes in client condition to supervision.
• Promote safe healthy environment through activities.
• Observe aseptic technique in procedures.
• Collaborate with clinical team to improve Plan of Care.
• Assist client in achieving goals in Plan of Care.
• Maintain productivity per agency standards.
• Maintain licenses and required training.
• Participate in meetings and training as required.
• Provide service per agency policies and scope of practice.

Homecare with Heart is offering you the most competitive pay scale in the area. We're looking for compassionate and experienced nurses to provide care for our clients in their homes. As a Home Care Nurse, you will be responsible for overseeing the quality, expert care of your assigned homecare patients. Starting rate for LPN: $27 per hour depending on experience.

Responsibilities

What You'll Do:

• Administer medication in accordance with standards of practice.
• Encourage social ehabilitative skills and encourage self-care and independence.
• Perform client treatments in accordance with Plan of Care
• Assist the consumer in learning appropriate self-care techniques.
• Prepare clinical and progress notes to document care provided.
• Monitor, document accordingly and report any change in client condition to Clinical Director/Clinical Supervisor
• Promote activities to achieve a safe and healthy environment.
• Observe aseptic technique when performing client procedures/care.
• Collaborate with other members of the clinical team to develop and improve the Plan of Care
• Assist the client in achieving established goals in the Plan of Care developed by the RN.
• Maintain productivity according to the standards of the agency.
• Maintain license, CEUs and mandatory employment requirements.
• Participate in client care meetings, in-services, orientation and staff meetings.
• Provide service in accordance with agency's philosophy, standards, policies and scope of practice
• Participates in Performance Improvement and adheres to HIPAA guidelines.
• Other duties as assigned.
