Job summary

Alternate Solutions Home Health, LLC is seeking a Licensed Practical Nurse (LPN) to provide skilled nursing care in patients’ homes. The role follows physician orders and RNCM guidelines, while educating patients and families and ensuring safety and compliance with regulations.

The LPN will monitor patient conditions, administer medications, participate in on‑call requirements, and document visits within Medicare guidelines.

Qualifications

• Licensed Practical Nurse with a current Ohio license.
• Minimum one year of LPN experience in an acute care setting (two years preferred).
• Home-care experience desired.
• Effective communication skills and ability to create positive impressions with patients, families, physicians, and co-workers.

Job Overview

Licensed Practical Nurse (LPN) located in Alternate Solutions Home Health. Schedule: Monday ‑ Friday, 8:15AM – 5:00PM with on‑call requirements. The LPN provides comprehensive skilled care ordered by the attending physician, follows order sets created by the RNCM, educates patients and family, ensures patient safety, maintains accurate documentation, and stays compliant with all regulations and state scope of practice.

Responsibilities

• Provide skilled nursing care in patients’ homes as ordered by the attending physician.
• Follow RNCM‑created order sets and goals.
• Observe and monitor patient conditions; notify RNCM of any changes.
• Administer medication as prescribed.
• Counsel patients and families on disease/injury processes and self‑management.
• Incorporate patient and family in the development of the plan of care.
• Teach preventive measures to help reduce re‑hospitalizations.
• Listen to patient and family members.
• Complete all clinical documentation in accordance with agency protocol and Medicare/Federal guidelines.
• Maintain an active LPN license in Ohio.
• Communicate with scheduler regarding any changes outside of normal hours.
• Participate in on‑call requirements and case conferences.
• Attend in‑service trainings and mandatory agency meetings.
• Maintain a professional appearance as a representative of the company.
• Understand and practice agency policies and procedures.
• Drive to patient’s primary location per scheduled visit.
• Attend all assigned visit locations; document visits within 24hours to meet Medicare guidelines.
• Complete and submit all required documentation within specified company requirements.
• Fulfill LPN visits in the assigned geographic location per patient needs within federal and state specifications.

• Licensed Practical Nurse with a current Ohio license.
• Minimum one year of LPN experience in an acute care setting (two years preferred).

• Calm, patient, accommodating, and compassionate while working with patients.
• Knowledgeable of nursing best practices; ability to make appropriate nursing judgments.

• Follow directions and work well as a team member.
• Valid driver’s license and auto insurance with your name listed as the driver.

Health & Physical Requirements

• Meet all physical position requirements.
• Provide evidence of a current negative tuberculin skin test or X‑ray at orientation; annual TB test thereafter unless documented otherwise.
• Free of medical conditions that would limit job function; may be exposed to bloodborne pathogens.
• Travel required; ability to read, speak, write English; use of computer/PDA; hearing, seeing, speaking, and physical abilities as outlined in the detailed physical demands list.
