Job summary

Jobtailor is seeking a qualified Registered Nurse with behavioral health experience to join our home health team in Plainfield, IL. You will assess patients’ physical, emotional, social, and environmental needs and develop individualized care plans coordinated with the care team.

Responsibilities include initial assessments, ongoing evaluations, monitoring progress, and educating patients and families about the plan of care.

Qualifications

• Current State unrestricted RN license.
• Graduated from an accredited Nursing program (Certificate, Diploma, ASN or BSN).
• At least 1 year of clinical nursing experience.
• Behavioral health nursing experience required.
• Home health experience preferred.
• Ability to lift up to 50 pounds and assist with transfers.
• Valid driver's license and reliable transportation to travel between patient homes.

Requirements

• 1+ year of experience as a nurse in a clinical care setting
• Must have Behavioral Health Nursing Experience
• Home Health experience is preferred, but not required
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations

Demonstrates expertise in Behavioral Health Nursing, including patient assessment, care plan development, and interdisciplinary communication. Capable of supervising staff and providing education on behavioral health care and symptom management.
