Job summary

Bradford Health Services is seeking an experienced Licensed Practical Nurse (LPN) to work in an assisted living setting. The role focuses on direct patient care, wound management, pain control, venipuncture, and patient education.

Collaboration with Nurse Practitioners and other healthcare professionals is essential to coordinate care plans. The ideal candidate holds an Alabama LPN license, demonstrates strong English communication, and has experience with wound care, venipuncture, and EHR

Qualifications

• Valid and current LPN license in Alabama.
• Excellent reading/writing/speaking English for patient communication.
• Experience in wound care and pain management.
• Ability to perform venipuncture safely.
• Experience in an assisted living facility or similar healthcare setting.

Responsibilities

• Provide comprehensive patient care including monitoring vital signs and administering medications.
• Assist with activities of daily living for residents.
• Perform wound care management and documentation.
• Conduct venipuncture procedures for testing.
• Educate patients and families on health maintenance and care plans.
• Collaborate with Nurse Practitioners and interdisciplinary teams.
• Maintain accurate patient records in line with policies.
• Manage pain through assessment and intervention to improve comfort.

About The Role

The Licensed Practical Nurse (LPN) plays a critical role in delivering high‑quality nursing care within an assisted living facility or similar healthcare setting. This position focuses on providing direct patient care, including wound management, pain control, and performing venipuncture, ensuring patients’ comfort and safety. The LPN will also be responsible for educating patients and their families about health conditions and care plans, promoting better health outcomes. Collaboration with Nurse Practitioners and other healthcare professionals is essential to coordinate care and implement treatment plans effectively. Ultimately, the LPN contributes to maintaining a supportive and healing environment that enhances the quality of life for all residents under their care.

Minimum Qualifications

• Valid and current Nursing License (LPN) in the Alabama.
• Proficiency in reading, writing, and speaking English to communicate effectively with patients and healthcare teams.
• Demonstrated experience in wound care and pain management.
• Ability to perform venipuncture safely and competently.
• Experience working in an assisted living facility or similar healthcare environment.

Preferred Qualifications

• Previous collaboration experience with Nurse Practitioners or advanced practice providers.
• Additional certifications in wound care or pain management.
• Experience with electronic health record (EHR) systems.
• CPR and Basic Life Support (BLS) certification.
• Continuing education in patient education techniques or geriatric nursing.

Responsibilities

• Provide comprehensive patient care including monitoring vital signs, administering medications, and assisting with daily living activities.
• Perform wound care management, including assessment, cleaning, dressing changes, and documentation.
• Conduct venipuncture procedures safely and accurately for laboratory testing and other medical needs.
• Educate patients and their families on health maintenance, disease prevention, and post‑treatment care instructions.
• Collaborate with Nurse Practitioners and interdisciplinary teams to develop and implement individualized care plans.
• Maintain accurate and timely patient records in compliance with regulatory standards and facility policies.
• Manage pain effectively through assessment, intervention, and documentation to improve patient comfort.
• Ensure compliance with all nursing licenses, facility protocols, and healthcare regulations.
