Job summary

Amedisys Home Health in Georgia is hiring a licensed practical nurse (LPN) to provide in-home patient care. You will verify plans of care with the RN, collaborate with physicians, teach self-care, monitor vitals, and perform required procedures during visits.

This full-time role offers mileage reimbursement, competitive base pay per visit ($31–$36), paid time off, telehealth options, and a comprehensive benefits package including health plans and a company 401(k) match.

Qualifications

• 1 year of clinical LPN experience.
• Current LPN license for the state(s) you work in.
• Current CPR certification.
• Valid driver’s license, reliable transportation and liability insurance.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN to ensure patient needs are met; notify changes promptly.
• Teach patients and caregivers self-care techniques.
• Check vital signs at visits.
• Review medications and perform medication reconciliation.
• Perform physician-ordered specialized procedures (e.g., foley catheter changes).
• Prepare equipment and materials for treatments using aseptic technique.
• Complete documentation timely and accurately.
