Job summary

Community Care Physicians is seeking a full-time LPN for Charlton Family Medicine of Ballston Lake. Hours are Monday-Friday, 7:30am-4:00pm. Eligible for a recruitment bonus up to $3,500.

You will administer vaccines, document conversations, and enter orders in the EMR while maintaining patient flow and confidentiality. Qualifications include NYS LPN license and prior practice experience; strong computer skills and familiarity with EMR are required.

Qualifications

• Valid NYS LPN license.
• Previous LPN experience in a medical practice or prior experience as a Medical Assistant or Patient Care Tech.
• Strong computer skills with experience using EMR.
• Knowledge of basic nursing principles and protocols.
• Knowledge of policies and procedures related to infection control, environmental safety, and patient confidentiality.
• Maintain strictest confidentiality and discretion.
• Knowledge of medical chart reporting/records.
• Skill in appropriate assessment and assistance techniques.

Our Charlton Family Medicine of Ballston Lake is looking for a Full Time LPN to join their team! The hours are: Monday-Friday, 7:30am-4:00pm.

This position is eligible for up to a $3,500 recruitment and retention bonus!!

Responsibilities:

• Administer and record vaccines.
• Document verbal conversations with patients when appropriate.
• Document information into patient record in an accurate, legible and timely manner.
• Maintain timely patient flow.
• Prepare patient for visit as appropriate, based on office/provider requirements.
• Obtain specimens (i.e., urine, blood, etc.) from patients as directed by the provider.
• Accurate and timely order entry into the lab system.
• Perform clinical office procedures as directed by the provider (i.e. PFT, EKG, nebulizer, medication administration and immunizations)
• Process patient prescriptions.
• Discuss test results with patients per provider's instructions.

Compensation:

Compensation: $26.00 - $33.00 per hour CCP salary ranges are designed to be competitive with room for professional and financial growth.
