Job summary

Charlton Family Medicine of Ballston Lake is seeking a qualified LPN for a full-time role. The position requires Monday through Friday, 7:30am to 4:00pm, with responsibilities including vaccine administration, patient documentation, and maintaining smooth patient flow.

The LPN will document conversations, obtain specimens, enter lab orders, and support clinical procedures under the provider's direction. A NYS LPN license and EMR proficiency are required for this role.

Qualifications

• Valid NYS LPN license is required.
• Previous LPN experience in a medical setting preferred.
• Strong computer skills and EMR proficiency are essential.

Responsibilities

• Administer and record vaccines.

Our Charlton Family Medicine of Ballston Lake is looking for a Full Time LPN to join their team! The hours are: Monday-Friday, 7:30am-4:00pm.

• This position is eligible for up to a $3,500 recruitment and retention bonus!!**

Responsibilities

• Administer and record vaccines.
• Document verbal conversations with patients when appropriate.
• Document information into patient record in an accurate, legible and timely manner.
• Maintain timely patient flow.
• Prepare patient for visit as appropriate, based on office/provider requirements.
• Obtain specimens (i.e., urine, blood, etc.) from patients as directed by the provider.
• Accurate and timely order entry into the lab system.
• Perform clinical office procedures as directed by the provider (i.e. PFT, EKG, nebulizer, medication administration and immunizations)
• Process patient prescriptions.
• Discuss test results with patients per provider's instructions.

Compensation: $26.00 - $33.00 per hour CCP salary ranges are designed to be competitive with room for professional and financial growth. Individual compensation is based on several factors unique to each candidate, such as work experience, qualifications, and skills. Some roles may also be eligible for overtime pay.
