Job summary

Haven of St. Elmo is seeking compassionate Licensed Practical Nurses (LPNs) to join our skilled nursing and long-term care team in Illinois. We offer supportive environment, consistent scheduling, and opportunities for advancement.

Nurses provide direct care, medications, and collaborative care with physicians and CNAs. Both full-time and part-time positions are available with wage rates ranging $32.00–$37.00 per hour.

Qualifications

• Active LPN license in Illinois is required.
• Graduate of an accredited nursing program.
• Experience in long-term care, skilled nursing, or rehabilitation is preferred.
• Ability to work every other weekend.
• Strong clinical judgment, communication, and organizational skills.
• Compassionate, dependable, and resident-focused.

Responsibilities

• Provide direct nursing care in a skilled nursing and long-term care setting.
• Administer medications, treatments, and IV therapy as prescribed.
• Assess, monitor, and document resident conditions and care plans.
• Collaborate with physicians, nurse practitioners, and interdisciplinary care teams.

• Ensure compliance with Illinois nursing standards and facility policies.
• Respond to changes in resident conditions and provide clinical leadership during your shift.
