Job Summary

We are hiring Licensed Practical Nurses (LPNs) in Illinois to join our team at a Haven Healthcare–affiliated skilled nursing and long‑term care facility in Grayville. Key Responsibilities (LPN Duties)

• Provide direct nursing care to residents in a skilled nursing / long‑term care setting
• Administer medications and treatments as prescribed
• Monitor patient conditions and report changes to the RN or physician
• Perform wound care and assist with care planning
• Supervise and support Certified Nursing Assistants (CNAs)
• Maintain accurate and timely clinical documentation
• Follow Illinois nursing standards, safety protocols, and infection control procedures

Qualifications
