Journey in Saint Albans, WV is seeking a dedicated Registered Nurse to lead the nursing team in a long-term care setting. You will oversee LPNs, CNAs, and care coordination to ensure high-quality resident outcomes, while maintaining regulatory compliance and a compassionate care culture.

You will participate in admissions and transfers, monitor resident status changes, and collaborate with physicians and families to execute care plans. A valid RN license and CPR certification are required.

Formación

• Nursing degree from an accredited program.
• Current RN license in practicing state.
• Current CPR certification.

Responsabilidades

• Directs daily activities of licensed practical/vocational nurses and CNAs in accordance with regulations and facility policies.
• Follows nursing model to promote holistic care for residents.
• Ensures policies and procedures are followed by nursing staff.
• Participates in admissions, discharges, and transfers of residents as required.
• Assesses changes in residents’ status and communicates with physicians and families; documents accordingly.
• Transcribes physician orders to medical records and carries out orders as written.

Major Duties and Responsibilities

• Directs the daily activities of the licensed practical/vocational nurse and certified nursing assistants in accordance with current federal, state and local regulations and guidelines and established facility policies and procedures.
• Follows the nursing model to promote holistic care for the residents.
• Ensures that policies and procedures are complied with by nursing personnel assigned.
• Participates in the admission, discharge and transfers of residents as required.
• Assesses for changes in residents’ status, notifying the physician and resident’s family or representative and documenting accordingly.
• Transcribes physician orders to medical record and carries out orders as written.

Required Qualifications

• Prior experience in long-term care preferred but not required.
• A Nursing degree from an accredited college or university.
• Current unrestricted license as a Registered Nurse (RN) in practicing state.
• Current CPR certification.
