Reliant Care Management Compan seeks an LPN for FT Nights to provide compassionate nursing care for residents in a supportive facility environment.

Responsibilities include assessments, admissions, discharges, wound care, and leading CNAs/CMTs while adhering to facility protocols. Commitment to teamwork and patient safety is essential.

Job Responsibilities and Duties:

• Perform admission duties: complete assessment, MAR, TAR, POS, review chart set-up, physician orders, and order medications

• Manage discharges: complete documentation, notify physician and guardian, and arrange transportation

• Provide wound care: follow protocols, irrigate, pack, and remove dressings

• Lead CNAs and CMTs, ensuring compliance with facility protocols and policies

• Respond effectively to emergency situations

• Will be working with Behavioral Health Residents
