Job summary

Diamond Care Group is expanding its home health team in Florida and seeks a dedicated Home Health LPN to provide compassionate patient care in the home setting. You will chart observations, administer medications, and support families with education and care planning.

The ideal candidate holds a valid LPN license in good standing, has experience in home health, and demonstrates strong communication, organization, and problem-solving skills. We offer a flexible schedule and mileage reimbursement.

Qualifications

Responsibilities

• Chart observations of the patient's condition at each visit.
• Administer Physician-prescribed medication.
• Dress or redress wounds and monitor the healing progress.
• Provide education to patients and families on proper home health care procedures and strategies.
• Coordinate with Physicians, Physical Therapists, Occupational Therapists and other individuals in the patient's care plan.
• Recommend tools or devices that may improve the quality of life for the patient.

Qualifications

• Valid LPN license and in good standing
• Proven experience as a licensed practical nurse, preferably in home health
• Excellent knowledge of nursing care methods and procedures
• In-depth knowledge of health and safety guidelines and procedures (sanitation, decontamination etc.) and willingness to always follow them
• A team player with excellent communication and interpersonal skills
