Job summary

Wexford Health Sources, Inc. is seeking a Licensed Practical Nurse (LPN) to support care at Sam Perdue Juvenile Center in Princeton, WV.

This PRN role offers shift options: 6:00 am–2:30 pm or 2:00 pm–10:30 pm, providing hands-on nursing under supervision of RNs and medical providers. You will administer medications, monitor vital signs, assist with activities of daily living, wound care, documentation, and participate in a multidisciplinary team to deliver compassionate, patient-centered care

Qualifications

• Active LPN license or equivalent.
• 1–2 years nursing experience; correctional experience preferred.
• Active CPR certification.
• Strong clinical and communication skills; adaptable in high-pressure settings.

Responsibilities

• Administer medications and treatments as ordered.
• Assist with activities of daily living and therapeutic interventions.
• Take vital signs and monitor patient conditions.
• Provide wound care and dressing changes.
• Support documentation and care planning.
• Promote hygiene, sanitation and a safe environment.

Position Summary
As an LPN at our facility, you’ll play a hands‑on role in delivering care to a vulnerable population under the guidance of RNs and medical providers. Some responsibilities include:

• Providing wound care and changing dressings

What You’ll Need

• Active unrestricted LPN license and 1-2 years of nursing experience (correctional experience preferred).
• Active CPR certification.
• Strong clinical and communication skills; adaptability in high-pressure settings.
