Job summary

Oak Park Place is seeking a Licensed Practical Nurse (LPN) to travel between our Oak Creek and Wauwatosa communities. The role is full-time and non-exempt, with responsibilities focused on resident care according to the Plan of Care and timely communication of health changes.

The Licensed Practical Nurse (LPN) is responsible for providing thoughtful care to residents and their families based on the personalized Plan of Care, monitoring and tracking changes in conditions and needs, and proactively updating the appropriate contacts on the plan of care, health changes, and concerns.
