Oak Park Place in Wisconsin is seeking a Licensed Practical Nurse (LPN) to provide thoughtful care to residents based on the personalized Plan of Care, monitoring changes and proactively communicating health updates to the appropriate contacts.

The LPN will monitor health conditions, document changes, and collaborate with the care team to ensure consistent, high-quality support for residents and their families.

Competenze

• Previous nursing or medical experience required.
• Familiarity with medical software and equipment.
• Ability to build rapport with patients.
• Must hold a current LPN license.
