Job summary

Oak Park Place in Wisconsin is seeking a dedicated Licensed Practical Nurse (LPN) to travel between our Oak Creek and Wauwatosa communities. You will provide thoughtful resident care based on personalized care plans, monitor health changes, and coordinate with family and care teams.

Qualifications

The Licensed Practical Nurse (LPN) is responsible for providing thoughtful care to residents and their families based on the personalized Plan of Care, monitoring and tracking changes in conditions and needs, and proactively updating the appropriate contacts on the plan of care, health changes, and concerns.
