Job summary

Community Care Physicians is seeking a Full Time LPN for our Family Practice in Ballston Spa. The position is 40 hours per week with specific schedules (Mon, Tue, Thu, Fri 7:30–4:30; Wed 7:00–1:00). A recruitment/retention bonus of up to $3,500 is offered.

Responsibilities include vaccines, documentation, patient flow, specimen collection, lab orders, and patient communication. Compensation ranges from $26–$33 per hour, with a comprehensive rewards package.

Qualifications

• Valid NYS LPN license.
• Experience as LPN in a medical practice or as Medical Assistant/Patient Care Tech.
• Strong computer skills with EMR experience.
• Knowledge of basic nursing principles and infection control.
• Knowledge of policies and procedures related to infection control, environmental safety, and patient confidentiality.
• Maintain strictest confidentiality and discretion.
• Knowledge of medical chart reporting/records.
• Skill in appropriate assessment and assistance techniques.

Responsibilities

• Document information into patient records accurately and legibly.
• Maintain timely patient flow.
• Prepare patient for visit as appropriate.
• Obtain specimens (urine, blood, etc.) as directed by the provider.
• Enter orders accurately into the lab system.
• Perform clinical office procedures as directed by the provider (e.g., PFT, EKG, nebulizer, medications, immunizations).
• Process patient prescriptions.
• Discuss test results with patients per provider's instructions.

This is a 40 hour week. The hours are: Monday, Tuesday, Thursday, Friday: 7:30 AM-4:30 PM | Wednesday: 7:00 AM-1:00 PM.

This position is eligible for up to a $3,500 recruitment and retention bonus!!

Responsibilities:

• Administer and record vaccines.
• Document verbal conversations with patients when appropriate.
• Document information into patient record in an accurate, legible and timely manner.
• Maintain timely patient flow.
• Prepare patient for visit as appropriate, based on office/provider requirements.
• Obtain specimens (i.e., urine, blood, etc.) from patients as directed by the provider.
• Accurate and timely order entry into the lab system.
• Perform clinical office procedures as directed by the provider (i.e. PFT, EKG, nebulizer, medication administration and immunizations)
• Process patient prescriptions.
• Discuss test results with patients per provider's instructions.

Compensation: $26.00 - $33.00 per hour
