Job summary

Community Care Family Medicine is seeking a Full Time LPN to join our team in Clifton Park, NY. This 40-hour-per-week role offers a competitive hourly rate and a recruitment/retention bonus of up to $3,500, with a broad benefits package.

Responsibilities include vaccines, patient record documentation, order entry, and clinical procedures under provider direction. Prior EMR experience and NY LPN license required.

Qualifications

• Valid NYS LPN license.
• Experience as LPN in a medical practice or as MA/PCT.
• Strong computer skills and EMR experience.
• Knowledge of infection control and patient confidentiality.

Responsibilities

• Administer and record vaccines.
• Document verbal conversations with patients when appropriate.
• Document information into patient records accurately and timely.
• Maintain timely patient flow.
• Prepare patients for visits per office/provider requirements.
• Obtain specimens (urine, blood, etc.) as directed by the provider.
• Enter orders into the lab system accurately and promptly.
• Perform clinical procedures as directed (PFT, EKG, nebulizer, meds, immunizations).
• Process patient prescriptions.
• Discuss test results with patients per provider's instructions.

Our Community Care Family Medicine officeis looking for a Full Time LPN to join their team!

This is a 40 hour per week position.

**This position is eligible for up to a $3,500 recruitment and retention bonus!!**

Responsibilities:

• Administer and record vaccines.
• Document verbal conversations with patients when appropriate.
• Document information into patient record in an accurate, legible and timely manner.
• Maintain timely patient flow.
• Prepare patient for visit as appropriate, based on office/provider requirements.
• Obtain specimens (i.e., urine, blood, etc.) from patients as directed by the provider.
• Accurate and timely order entry into the lab system.
• Perform clinical office procedures as directed by the provider (i.e. PFT, EKG, nebulizer, medication administration and immunizations)
• Process patient prescriptions.
• Discuss test results with patients per provider's instructions.

Compensation: $26.00 - $33.00 per hour
