Job summary

Amedisys is seeking a licensed practical nurse to join our home health team in Medford, NY. This full-time role involves visiting patients, coordinating with RNs and physicians, and delivering high-quality patient care in the home setting.

Qualifications

• One year of clinical LPN experience.
• Current LPN license, specific to the state(s) assigned.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN frequently to ensure patient needs are met; notify them promptly if patient condition changes.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review all medications and perform a medication reconciliation at each visit.
• Perform specialized procedures ordered by physician (e.g. foley catheter change).
• Prepare equipment and materials for treatments using aseptic technique.
• Complete documentation timely and accurately.
