Bluegrass Care Navigators in Kentucky is seeking a Licensed Practical Nurse (LPN) to provide hospice nursing in patients' homes, nursing facilities, or other settings.
Under RN supervision, you will deliver care per the plan, monitor patient status, educate caregivers, and document in EMR. Schedule Monday–Friday, 8:00am–5:00pm with travel across Fayette and Jessamine counties.
As a Bluegrass Care Navigators' Hospice Team member, you will be a part of a team providing care and support to patients and their families who no longer receive treatments to cure their terminal illness but instead choose care to improve their comfort and quality of life. Our service allows our team members to focus on one patient at a time providing care where the patient calls home. While this often is a house or apartment, we also provide care at nursing homes, assisted living facilities, and hospitals. If a patient’s care needs are too complicated for home care or require a higher level of attention from nurses and other experts, we can provide care at one of our Hospice Care Centers.

The Licensed Practical Nurse (LPN) provides skilled and compassionate nursing care to hospice patients in their place of residence, including private homes, nursing homes, assisted living facilities, or other settings the patient calls home. Working under the direction and supervision of a Registered Nurse, the LPN delivers individualized care in accordance with the established plan of care to promote patient comfort, safety, and quality of life at the end of life. The LPN is responsible for performing assigned nursing and personal care tasks, monitoring and reporting changes in patient condition, educating patients and caregivers, and documenting care accurately and timely within the electronic medical record (EMR).
