Job summary

Bluegrass Care Navigators is seeking a Licensed Practical Nurse to provide skilled nursing care to hospice patients in their homes and other settings. You will work under a Registered Nurse to follow the care plan and promote comfort and quality of life at the end of life.

You will document care in the EMR, educate patients and caregivers, and monitor changes in patient condition while adhering to professional standards and the hospice philosophy.

Qualifications

• Graduate of an accredited practical nurse or vocational nursing program.
• Current LPN licensure in good standing with the Kentucky Board of Nursing.
• Minimum of two (2) years of nursing experience; community health/hospice, medical-surgical, or long-term care experience preferred.
• Demonstrates compliance with accepted professional nursing standards and practices.
• Understands and supports the philosophy of hospice care and the needs of terminally ill patients.
• Must be a licensed driver with an automobile in good working order, that is insured in accordance with state minimum requirements.

Responsibilities

• The LPN provides skilled and compassionate nursing care to hospice patients in their place of residence, including private homes, nursing homes, assisted living facilities, or other settings the patient calls home.
• Works under the direction and supervision of a Registered Nurse, delivering individualized care in accordance with the established plan of care to promote patient comfort, safety, and quality of life at the end of life.
• Performs assigned nursing and personal care tasks, monitors and reports changes in patient condition, educates patients and caregivers, and documents care accurately and timely within the EMR.
