Lacey Post Acute & Rehabilitation in Washington is seeking dedicated LPNs for full-time, part-time, and per diem roles. Competitive pay with shift differentials, day pay options, and strong benefits are offered.

The position supports a team-focused environment in a top-rated long-term care and skilled nursing facility. As an LPN, you will administer medications, monitor residents, and collaborate with RNs, CNAs, and other professionals to ensure high-quality care.

Qualifications

• Licensed Practical Nurse (LPN) license current in the state.
• CPR/BLS certification (or willing to obtain).
• Previous LTC or SNF experience preferred, but new grads welcome.

Responsibilities

• Provide nursing care to residents per standards of practice and physician orders.
• Administer medications and treatments with precision and compassion.
• Collaborate with RNs, CNAs, and interdisciplinary teams.
• Support residents’ physical, emotional, and psychological well-being.
• Assist with resident comfort and comfort measures.
• Collect samples and monitor catheters; provide wound care.
• Maintain confidentiality and follow all policies and regulations.
• Track patient history, vital signs, and physical condition; support staff as needed.

What You'll Do

Provide nursing care to ensure resident needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines. Administer medications and treatments with precision and compassion.

• Support residents’ physical, emotional, and psychological well-being
• Assist with resident comfort.
• Collect samples and monitor catheters. Provide wound care.
• Keep resident patient information confidential, follow all policies and procedures, and adhere to federal, state, local requirements, and professional standards.
• Collect, track, maintain, and review patient medical history, charts, and physical condition, including heart rate, blood pressure, temperature, and respiration.
• Help coach staff as necessary.

Requirements

• Current LPN license in [State]
• CPR/BLS certification (or willing to obtain)
• Previous LTC or SNF experience preferred, but new grads welcome!
• Positive attitude, reliability, and strong clinical judgment
