Job summary

Amedisys in Port Charlotte, FL is hiring a licensed practical nurse (LPN) for full-time home health visits. Verify plan of care with RN, collaborate with physicians, teach patients and caregivers self-care, monitor vital signs, and perform medication reconciliation.

Qualifications

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN frequently to ensure patient needs are met.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review medications and perform medication reconciliation at each visit.
• Perform specialized procedures ordered by physician (e.g. foley catheter change).
• Prepare equipment and materials for treatments using aseptic technique.
• Complete documentation timely and accurately.

• Complete documentation timely and accurately.

Qualifications

• Valid driver's license, reliable transportation and liability insurance.
