Job summary

Amedisys is hiring licensed practical nurses to provide in-home patient care across the Carlisle/Mechanicsburg area. The role involves coordinating with RNs and physicians, performing routine assessments, and delivering patient education in a home setting.

Ideal candidates have an active LPN license in the relevant state, CPR certification, and dependable transportation. Full-time daytime hours with benefits are offered.

Qualifications

• One year of clinical LPN experience is required.
• Current LPN license for the state(s) you work in.
• CPR certification is required.
• Valid driver’s license with reliable transportation.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN to meet patient needs and report changes.
• Teach patients and caregivers self-care techniques.
• Check vital signs and perform medication reconciliation at visits.
• Prepare equipment for treatments using aseptic technique.
• Complete documentation timely and accurately.

Responsibilities

• Collaborate with physician and RN frequently to ensure patient needs are being met; Notify them as soon as possible when patient condition changes.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review all medications and perform a medication reconciliation at each visit.
• Perform specialized procedures ordered by physician (i.e. foley catheter change).
• Prepare equipment and materials for treatments using the aseptic technique.
• Complete documentation timely and accurately.

Qualifications

• One year of clinical LPN experience.
• Current LPN license, specific to the state(s) you are assigned to work.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.
