Grady Health System in Atlanta is seeking dedicated nursing professionals to support assessment, planning, implementation, and evaluation of individualized care plans. The role emphasizes collaboration with nurses and physicians and adherence to required Life Support standards and licensure.

ASSESSMENT – Participates in the collection and analysis of data related to care delivery and reports critical findings to the Registered Nurse and/or Physician
PLANNING and COLLABORATION– Participates in the development of the individualized plan of care that identifies strategies and alternatives to attain expected outcomes and collaborates with other members of the healthcare team as appropriate
IMPLEMENTATION – Participates in the execution of the individualized patient/family plan
EVALUATION – Participates in the evaluation process forward
AGHBasic Life Support Course - AHA - American Heart Association required
Georgia Practical Nurse License - Georgia Secretary Of State required
