Job summary

Community Health Services is seeking a dedicated LPN to join our Fremont, OH team. You will care for patients Monday through Friday, performing assessments and coordinating with providers.

Must be an accredited Nursing Program graduate with an Ohio LPN license and BLS certification. Prior experience with EHR and strong communication are required for success.

Qualifications

• Graduate of a current accredited Nursing Program.
• Current License to practice in the State of Ohio as an LPN.
• Current Basic Life Support Certification.
• Minimum of one year as an LPN preferred.
• Knowledge of documenting with EHR.

Responsibilities

• Work directly with patients Monday through Friday- No Weekends.
• Perform Adult and Pediatric Nursing Assessments.
• Communicate clearly with Providers, Patients, and CHS Staff.
• Follow-up on Lab and Diagnostic testing.
• Administer injections as needed.
• Follow all safety, HIPAA, and regulatory guidelines.

Hours for this position are:

Mondays 7am-7pm, Tuesdays through Thursdays 8am-5pm, Fridays 8am-1pm

What You’ll Do:

• Work directly with patients Monday through Friday- No Weekends

What We’re Looking For:
