Community Health Services in Fremont, OH is seeking a full-time Family Nurse Practitioner to provide care for adults and children at 2221 Hayes Avenue. The position emphasizes compassionate, patient-centered care in a federally qualified health center.

You will work Monday through Friday with no weekends, perform adult and pediatric assessments, coordinate with providers, and ensure compliance with HIPAA and safety guidelines. NHSC loan repayment available up to $75,000 for full-time staff.

Qualifications

• Licensed to practice in Ohio as a Family Nurse Practitioner.
• Basic Life Support (BLS) certification.

Salary Range: $120,000.00

Full-time Family Nurse Practitioner at Community Health Services – 2221 Hayes Avenue in Fremont. Outstanding opportunity for an experienced, compassionate FNP who enjoys caring for a diverse population of patients, including both adults and children.

Applicants must have at least 2 years of experience in a family practice setting.

$120,000 base salary, no RVU's and paid malpractice insurance. As a Federally Qualified Health Center, providers are eligible to apply for student loan repayment through National Health Service Corps (NHSC) – up to $75,000 for full-time employees. Other benefits include paid vacation, sick time, and retirement benefits.
