Community Health Services in Fostoria, OH is seeking a licensed practical nurse (LPN) to join a dedicated team. Qualifications

• Graduate of an accredited Nursing Program.
• Current License to practice in the State of Ohio as an LPN.
• Current Basic Life Support Certification.
• Minimum of one year as an LPN preferred.
• Knowledge of documenting with EHR.
• Strong communication skills and the ability to multitask.
• Ability to think critically and solve problems.

Responsibilities

• Assess patients under Nursing supervision and perform adult/pediatric assessments.
• Conduct point-of-care testing and follow up on lab results and diagnostics.
• Administer injections as needed and ensure patient safety and HIPAA compliance.
• Coordinate with providers, CHS staff, and patients to support quality care.
