Provide direct patient care including medications, vital signs, treatments, and daily living support
Develop and implement individualized care plans in collaboration with the interdisciplinary team

Conduct assessments and adjust care plans as needed
Collaborate with healthcare professionals to ensure seamless care transitions
Maintain accurate and detailed documentation of care and treatments
