LPN 7am-3pm
Job Summary:
At Liberty Center for Rehabilitation and Nursing, we pride ourselves on "Outstanding medical and rehabilitation services". We are seeking a dedicated and compassionate LPN to join our team, working with us to provide exceptional care to our patients. As an LPN on our Day Shift, you will play a vital role in our nursing staff, working a variety of shifts, including days, evenings, and weekends.
Responsibilities:
- Provide high-quality patient care under the supervision of the licensed nurse, including administering medications, performing treatments, and maintaining accurate records
- Develop and implement individualized patient care plans, in collaboration with the interdisciplinary team
- Participate in care conferences and contribute to the development of patient goals and objectives
- Maintain a safe and clean environment for patients, visitors, and staff
- Communicate effectively with patients, families, and caregivers to ensure seamless care coordination
- Perform routine checks on patients, including vital signs, and report any changes or concerns to the licensed nurse
Requirements:
- Graduate of an approved LPN program with a valid license to practice as an LPN in the state of Pennsylvania
- Minimum of 0 years of experience as an LPN, with a willingness to learn and grow with our team
- Strong communication and interpersonal skills, with the ability to work effectively in a fast-paced environment
- Ability to lift, transfer, and reposition patients, as needed
Working Conditions:

- Work in a fast-paced, dynamic environment, with frequent interruptions and changes in priorities
- Participate in on-call rotation as needed
