Job summary

Millennium Physician Group is seeking Licensed Practical Nurses (LPN) to join our Home Health Team across Charlotte, Collier, Lee and Sarasota Counties. This PRN role provides skilled nursing care under the supervision of a Registered Nurse, performing selected tasks in curative, rehabilitative, palliative, or preventive care.

Per diem schedule offers flexible hours; ideal candidates have Florida LPN licensure, at least 6 months of clinical experience, and strong communication and teamwork.

Qualifications

• Current and valid LPN license in Florida.
• Minimum of 6 months of clinical LPN experience in a hospital or skilled nursing facility.
• Excellent communication, interpersonal, and critical thinking skills.
• Strong work ethic and reliability.
• Reliable transportation to visit patients at home.

Responsibilities

• Provide high-quality nursing care, including administering medications, wound care, and vital sign monitoring.
• Conduct comprehensive assessments to identify patient needs and develop individualized care plans.
• Accurately administer medications and educate patients on their medication regimen.
• Implement prescribed treatments, such as IV therapy and catheter care.
• Provide patient and family education on disease management, prevention, and self-care.
• Collaborate with physicians, therapists, and other healthcare providers to ensure optimal patient outcomes.
