Millennium Physician Group is seeking a Registered Nurse (RN) to join our Home Health Team serving Charlotte, Collier, Lee, and Sarasota Counties. The PRN role offers flexible hours with potential varying schedules based on staffing needs, patient volume, and vacations.

You will conduct patient assessments, develop care plans, educate patients and families, and collaborate with the care team while ensuring compliant documentation.

Qualifications

• Active Florida RN license.
• Minimum 1 year experience in a hospital or skilled nursing facility.
• Strong clinical and communication skills with a patient-centered approach.

Responsabilités

• Perform comprehensive patient assessments including vital signs, wound care, and disease management.
• Develop individualized care plans addressing patient needs and goals.
• Educate patients and families on disease management and self-care techniques.
• Provide direct patient care including wound care, injections, and IV therapy as needed.
• Collaborate with physicians, therapists and other healthcare professionals to ensure coordinated care.
• Maintain accurate and timely documentation of assessments, interventions, and progress notes.
• Adhere to HIPAA and Medicare regulations.

Job Description Summary

We are seeking Registered Nurses (RN) to join our Home Health Team in Charlotte, Collier, Lee, and Sarasota Counties. In this role, there is a real impact in the comfort of patients’ homes. As part of our compassionate, high-performing care team, you’ll provide skilled, patient-centered home health care through assessments, care planning, clinical interventions, patient education, and collaboration with the care team while maintaining accurate documentation and compliance with regulatory standards.

This is a Per Diem (PRN) position with variable time and hours can vary from week to week based on staffing needs, patient volume, call-offs, vacations, or organisational requirements.

How will you make an impact & Requirements

What You’ll Do

• Patient Assessment: Conduct comprehensive assessments of patients' physical, mental, and emotional health, including vital signs, medication administration, wound care, and disease management.
• Care Planning: Develop individualized care plans that address patients' specific needs and goals, including medication administration, wound care, and disease management.
• Patient Education: Educate patients and their families on disease management, medication administration, and self-care techniques to promote independence and improve health outcomes.
• Clinical Interventions: Provide direct patient care, including wound care, medication administration, injections, and IV therapy.
• Collaboration: Collaborate with physicians, therapists, and other healthcare professionals to ensure coordinated and comprehensive care.
• Documentation: Maintain accurate and timely documentation of patient assessments, interventions, and progress notes.
• Compliance: Adhere to all relevant regulatory standards, including HIPAA and Medicare regulations.

What We’re Looking For

• Licensure: Current and valid Registered Nurse (RN) license in the state of Florida.
• Experience: Minimum of 1 year of experience in a hospital or skilled nursing facility.
• Skills: Strong clinical skills, excellent communication and interpersonal abilities, and a patient-centered approach to care.
• Professionalism: Strong work ethic, reliability, and a commitment to providing high-quality care.
• Transportation: Reliable transportation to visit patients in their homes.
