Job summary

Millennium Physician Group is seeking a Registered Nurse (RN) to join our Home Health Team across Charlotte, Collier, Lee, and Sarasota Counties in Florida. You’ll deliver skilled, patient‑centered home health care through assessments, care planning, interventions, patient education, and careful documentation while complying with HIPAA and Medicare regulations.

Qualifications

• Licensure: Current and valid RN license in Florida.
• Experience: Minimum 1 year in a hospital or skilled nursing facility.

Responsibilities

• Assess patients' physical, mental, and emotional health, including vital signs, meds, wound care, and disease management.
• Develop individualized care plans addressing needs and goals, including med administration, wound care, and disease management.
• Educate patients and families on disease management, medications, and self-care techniques.

Job Description Summary

We are seeking Registered Nurses (RN) to join our Home Health Team in Charlotte, Collier, Lee, and Sarasota Counties. In this role, there is a real impact in the comfort of patients’ homes. As part of our compassionate, high‑performing care team, you’ll provide skilled, patient-centered home health care through assessments, care planning, clinical interventions, patient education, and collaboration with the care team while maintaining accurate documentation and compliance with regulatory standards.

This is a Per Diem (PRN) position with variable time and hours can vary from week to week based on staffing needs, patient volume, call-offs, vacations, or organizational requirements.

How Will You Make An Impact & Requirements

What You'll Do

• Patient Assessment: Conduct comprehensive assessments of patients' physical, mental, and emotional health, including vital signs, medication administration, wound care, and disease management.
• Care Planning: Develop individualized care plans that address patients' specific needs and goals, including medication administration, wound care, and disease management.
• Patient Education: Educate patients and their families on disease management, medication administration, and self-care techniques to promote independence and improve health outcomes.
• Clinical Interventions: Provide direct patient care, including wound care, medication administration, injections, and IV therapy.
• Collaboration: Collaborate with physicians, therapists, and other healthcare professionals to ensure coordinated and comprehensive care.
• Documentation: Maintain accurate and timely documentation of patient assessments, interventions, and progress notes.
• Compliance: Adhere to all relevant regulatory standards, including HIPAA and Medicare regulations.

What We're Looking For

• Licensure: Current and valid Registered Nurse (RN) license in the state of Florida.
• Experience: Minimum of 1 year of experience in a hospital or skilled nursing facility.
