PARAMOUNT PREMIER HOMECARE LLC is seeking a compassionate Licensed Practical Nurse (LPN) to join our home care team in Bowmansville, NY. You will deliver high-quality, personalized care in patients' homes, collaborating with RNs and physicians to implement care plans and educate families.

Qualifications

• Active LPN license in New York.
• Home care, long-term care, or clinical nursing experience preferred.
• Knowledge of nursing procedures, infection control, and patient safety protocols.
• Strong communication with patients and families.
• Ability to work independently in home care.
• Valid driver’s license and reliable transportation.
• CPR/BLS certification preferred.

Responsibilities

• Provide direct patient care including wound care, medication administration, and vital sign monitoring.
• Assess and document patient health status, progress, and changes in condition.
• Collaborate with RNs, physicians, and care team to implement individualized care plans.
• Educate patients and family on care procedures, medications, and health management.
• Perform skilled nursing tasks in a home care setting per physician orders.
• Maintain accurate and timely clinical documentation and patient records.
• Ensure patient safety and comfort while promoting independence and dignity.
