Responsibilities include monitoring vital signs, maintaining documentation, collaborating on care plans, and educating patients and families.

Qualifications

• Current New York State LPN license in good standing.
• Minimum of one year of experience as an LPN in a clinical setting, preferably in home care.
• Strong communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Reliable transportation and a valid driver’s license.
• Ability to lift up to 50 pounds.
