Job summary

New Vanderbilt Rehabilitation and Care Center is seeking dedicated Licensed Practical Nurses (LPNs) to join our skilled nursing and rehabilitation facility. We provide sub-acute medical care, rehabilitation services, and 24-hour nursing care in a modern, secure environment with a supportive team.

Qualifications

  • LPN license valid in New York.
  • CPR certification required.

Responsibilities

  • Assess and document resident progress in the medical record.
  • Monitor turning and positioning of residents.
  • Administer medications and treatments as ordered by physicians.
  • Communicate resident status to charge nurses and supervision.
  • Maintain clean treatment areas and restock supplies.
  • Follow Infection Control policies and procedures.
  • Support education for residents and families when discharging to home.
  • Participate in Quality Improvement activities as assigned.

Summary of theLicensed Practical Nurse (LPN):

Provide nursing care to residents as prescribed by the physician and in accordance with standards of nursing practice and regulations and as directed by the Director of Nursing.

Essential Duties & Responsibilities of Licensed Practical Nurse (LPN):

  • Assess and document resident progress in the medical record.
  • Monitor turning and positioning of residents.
  • Obtain changes in physician’s orders as indicated by resident’s progress or lack of progress
  • Communicate pertinent information about resident condition and progress to charge nurses, supervisors and Director of Nursing, as indicated.
  • Maintain clean, orderly treatment cart and restock treatment supplies.
  • Follow Infection Control policies and procedures.
  • Administer medications and treatments accurately as ordered by the physician and according to all facility policies and procedures.
  • Document pressure sore status weekly or more often as condition changes.
  • Assist in monitoring the inventory of medications, medical supplies and equipment to be sure adequate supplies of skin care products are on hand to meet the needs of residents.
  • Participate in educational programs for residents and their family members relative to skin care when a resident is being discharged to home.
  • Participate in Quality Improvement activities, as assigned.

Job Requirements ofLicensed Practical Nurse (LPN):

  • Licensed Practical Nurse current state license in New York.
  • At least one (1) year of Skilled Nursing experience preferred.
  • Must have a current/active CPR certification.
  • Excellent technical, assessment, and documentation skills.
  • Effective verbal and written English communication skills.
  • Customer service oriented with the ability to work well under pressure.
  • Ability to work with minimal supervision, take initiative and make independent decisions.
  • Approachable, flexible and adaptable to change.
