BrightStar Care in Massachusetts seeks a Licensed Practical Nurse (LPN) to deliver compassionate home health care. You will work with a supportive team to care for clients in their homes, performing medications, monitoring health, and assisting with daily living tasks.

The role requires an active MA LPN license, at least 1 year of home health experience, and current BLS certification. Travel to patient homes is expected. EHR documentation and strong communication are essential.

Qualifications

• Minimum 1 year of home health or related healthcare experience
• Current BLS certification
• Reliable transportation and willingness to travel to patient homes
• Proficiency with electronic health records (EHR)
• Strong communication and interpersonal skills

Responsabilités

• Administer medications and treatments as prescribed by healthcare providers
• Monitor and document patient vital signs and overall health status
• Assist with daily living activities such as bathing, grooming, and meal preparation
• Educate patients and families on managing health conditions and care plans
• Coordinate with RNs, therapists, physicians, and other healthcare professionals
• Maintain accurate and timely documentation using EHR systems
• Provide emotional support and companionship
• Follow all safety and health regulations in home care settings
