Job summary

Quality Talent Group is seeking an Licensed Practical Nurse (LPN) to join a trusted network of skilled nursing and rehabilitation centers in New York Blue Point. You will provide direct nursing care, administer medications, and help implement care plans for residents in a team‑oriented, patient‑first environment.

Active LPN licensure is required; new grads are encouraged to apply. Experience with EHR and strong communication ensures high-quality care and collaboration with families and staff.

Qualifications

• Active and unrestricted LPN license in the state of practice.
• Recent graduates are encouraged to apply.
• Skilled in medication administration and patient care procedures.
• Strong documentation, time management, and communication skills.
• Comfortable using EHR and digital charting systems.

Responsibilities

• Deliver direct nursing care to residents in skilled nursing and long-term care settings.
• Administer medications and IVs as prescribed.
• Implement and evaluate individualized care plans.
• Record vital signs and monitor patient conditions.
• Supervise and guide other nursing personnel when required.
• Educate patients, families, and caregivers as directed.
