Job summary

Latham Medical Group, part of Community Care Physicians, is seeking an enthusiastic LPN to join their family medicine team in Latham, NY. The role offers a recruitment/retention bonus and a collaborative, learning-focused environment within CCP.

Schedule Monday–Friday with no nights or weekends, flexible hours, and paid holidays. Compensation ranges from $26.00 to $33.00 per hour, with comprehensive benefits in a modern multi-specialty setting near Albany.

Qualifications

• Valid NYS LPN license.
• Previous LPN experience in a medical practice preferred.
• Strong computer skills with EMR proficiency.
• Knowledge of nursing principles and infection control protocols.
• Ability to maintain confidentiality and document accurately.

Responsibilities

• Administer and record vaccines.
• Document verbal conversations with patients.
• Enter information into patient records accurately.
• Maintain timely patient flow and prepare patients for visits.
• Obtain specimens as directed by the provider.
• Enter orders in the lab system.
• Assist with clinical procedures as directed (PFT, EKG, nebulizer, etc.).
• Process patient prescriptions.
• Discuss test results with patients as instructed.

Centrally located, multi‑specialty environment:

Latham Medical Group is located in CCP’s Wellness Way health park in Latham, NY, opened in 2024, offering a modern, thoughtfully designed environment—just minutes from Albany and surrounding Capital Region communities.

Responsibilities

• Administer and record vaccines.
• Document verbal conversations with patients when appropriate.
• Document information into patient record in an accurate, legible and timely manner.
• Maintain timely patient flow.
• Prepare patient for visit as appropriate, based on office/provider requirements.
• Obtain specimens (i.e., urine, blood, etc.) from patients as directed by the provider.
• Accurate and timely order entry into the lab system.
• Perform clinical office procedures as directed by the provider (i.e. PFT, EKG, nebulizer, medication administration and immunizations)
• Process patient prescriptions.
• Discuss test results with patients per provider's instructions.

Compensation

$26.00 - $33.00 per hour

CCP salary ranges are designed to be competitive with room for professional and financial growth. Individual compensation is based on several factors unique to each candidate, such as work experience, qualifications, and skills. Some roles may also be eligible for overtime pay.

• Previous LPN experience in a medical practice or prior experience as a Medical Assistant or Patient Care Tech.
• Strong computer skills with experience using EMR.
• Knowledge of basic nursing principles and protocols.
• Knowledge of policies and procedures related to infection control, environmental safety, and patient confidentiality.
• Maintain strictest confidentiality and discretion.
• Knowledge of medical chart reporting/records.
