Job summary

CHRISTUS Health in Sulphur Springs, TX seeks a Licensed Physical Therapy Assistant to implement treatment programs under the supervision of the Physical Therapist and participate in department activities, safety and infection control.

You will apply individualized plans, document patient progress, assist discharge planning, supervise Rehab Technicians and students, and support scheduling and community referrals in a per diem setting.

Qualifications

• Associates degree or PTA certificate required.
• CPR certification required.
• Texas PTA license required.

Responsibilities

• Implements individualized treatment plans as indicated, considering diagnosis and age-specific needs, as established by the Physical Therapist in conjunction with the patient/family.
• Maintains quality documentation regarding patient status, to include: Daily and Weekly Progress Notes as required, and other notations as appropriate.
• Assists the physical therapist with discharge planning, including home programs, patient and family education/training, and identification of equipment needs.
• Assists in the supervision of Rehab Technicians, Physical Therapy Assistant students, and Volunteers.
• Participates in the Orientation of new Associates and scheduling of patients to assure quality patient coverage.
• Demonstrates adherence to the CORE values of CHRISTUS Santa Rosa.
• Understands and supports Performance Improvement activities.
• Complies with departmental productivity standards.
• Maintains an awareness of other agencies within the community in order to make appropriate referrals, when indicated, to network and to increase knowledge of community resources.
• Identifies and pursues educational opportunities for self and department.
• Participates in in-service programs, departmental and interdepartmental meetings.
• Demonstrates competence to perform assigned patient care responsibilities in a manner that meets the population-specific and developmental needs of patients served by the department.
• Appropriately adapts assigned patient assessment, treatment and/or methods to accommodate the unique physical, psychological, cultural, age-specific and other developmental needs of each patient served.
• Performs other related duties as directed.
