Job summary

Continuum Healthcare Inc. is seeking a Licensed Physical Therapist Assistant (PTA) to support the plan of treatment for residents within an interdisciplinary team.

The PTA will assist with rehabilitation, documentation, and coordination of care on-site and may travel to other facilities as needed. Qualified candidates hold an associate's degree in physical therapy or occupational therapy and are licensed or certified in the designated state.

Qualifications

• Graduate with an associate's degree in physical therapy or occupational therapy
• Licensed/certified in the designated state
• Knowledge of older adult rehabilitation

Responsibilities

• Provides resident rehabilitation care per plan of care
• Communicates with supervising therapist about resident condition and responses
• Completes required documentation accurately
• Provides feedback to therapists and Rehab Director
• Participates in center directed processes and services
• Demonstrates effective management of treatment and non-treatment activities
• Performs documentation to meet center, state, federal, and accrediting requirements
• Assists with marketing efforts as directed
• May travel to other facilities to provide treatments
