Job summary

Continuum Healthcare Inc. is seeking a licensed Physical Therapist Assistant (PTA) to support resident rehabilitation plans within an interdisciplinary team.

The PTA will work under the supervision of a licensed therapist to implement treatment plans and document patient responses, ensuring quality outcomes in our center setting. The role emphasizes collaboration with rehabilitation staff, adherence to regulatory standards, and possible travel to other facilities to provide treatments.

Qualifications

• Graduate with an associate degree in physical therapy or occupational therapy.
• Licensed or certified as PTA in the designated state.
• Experience in older adult rehabilitation preferred.
• Knowledge of governmental and regulatory guidelines in healthcare settings preferred.

Responsibilities

• Provides resident rehabilitation care within the plan of care and standard practices.
• Works closely with the supervising therapist to communicate resident condition and responses.
• Performs necessary documentation as indicated and directed.
• Provides feedback to the therapist and Director of Rehabilitation on clinical and rehabilitation issues.
• Participates in center-directed processes and services.
• Manages treatment and non-treatment related activities effectively.
• Performs documentation to meet center, state, federal, and accrediting requirements.
• Assists with marketing efforts as directed.
• May travel to other facilities to provide treatments.
• Performs other duties as required.
