Job summary

Focused Post Acute Care Partners FPACP is seeking a licensed Physical Therapist to join its Therapy department in Center, TX. You will evaluate patients, develop PT treatment programs with the physician, and administer a range of therapies to improve daily living activities.

You will supervise the Physical Therapy Assistant if assigned, coordinate with other clinicians, document progress, and ensure care meets state and federal standards while upholding FPACP's ROCKStar culture.

Qualifications

• Bachelor's Degree, Master's Degree, or Doctorate Degree in Physical Therapy from an accredited college or university.
• Current license/registration by state of hire as a Physical Therapist.

Responsibilities

• Establish, implement, and ongoing evaluation of treatment plans, goals, and discharge plans for PT needs in collaboration with the physician.
• Provide direct patient care services to ensure effectiveness of Physical Therapy programs.
• Apply heat, light, water, and electrically based treatments/modalties (e.g., ultrasound, diathermy) as prescribed.
• Prioritize, plan, and coordinate treatment and travel times to maximize efficiency while meeting patient needs.
• Ensure orders are carried out in a professional manner meeting ethics, patient rights, and regulations.
• Facilitate effective communication among clinicians and coordinate the PT program with other health care providers.
• Participate in interdisciplinary patient care conferences and maintain ongoing communication with the care team.
